The NHS is seeking your views on two issues, audiology services and end of life care services.
A new audiology patient survey has been launched by NHS Shropshire, Telford and Wrekin Clinical Commissioning Groups (CCGs) to gather patients’ views on the current service and how it may be improved.
The aim of the survey is to assess different areas of the adult and children audiology services including the care received by patients, aftercare, waiting times and access to appointments.
It will also identify any potential improvements which could be made to the services to enhance patient experience.

End of Life Care Services
The NHS is seeking answers to two questions relating to your expectations of end of life care services. End of Life, EOL, care in Shropshire, Telford and Wrekin relates to someone who is in or approaching the last 12 months of life. You do not need to be in this situation to answer the questions.
